If you have a terminal illness, hospice is here for you. Hospice staff will assess your individual needs and create an individual plan of care. Hospice staff is on-call 24 hours a day, seven days a week and focuses on supportive care and pain relief during the last period of an individual’s life. Hospice services are provided in your home, hospice centers, hospitals, nursing homes, long-term care facilities or free standing hospice centers.

Hospice staff:

  • manages the individual’s pain

  • provides medical and personal care services to the individual

  • educates family members on care for the individual;

  • assists the individual and his/her family members with the emotional, psychosocial and spiritual aspects of dying

  • provides needed medications, medical supplies, and equipment

  • arranges for additional services when needed - including respite care, speech and physical therapy, or inpatient care

  • provides grief support to family and friends.

Medicare pays for hospice care. A physician needs to certify that the disease is life threatening and likely to cause death within 6 months if the disease runs its normal course. Medicaid pays for hospice care. Many private insurance plans, HMOs, and other managed care organizations will pay for hospice care. There is no out of pocket cost. Donations support the cost of non-funded care.

When You Are Unable To Care For Yourself:

Florida law addresses the two most common documents through which individuals may make their health care wishes known:
-Living Will
-Designation of Health Care Surrogate

Five Wishes has become America's most popular living will because it is written in everyday language and helps start and structure important conversations about care in times of serious illness.

Five Wishes lets your family and doctors know:
-Who you want to make health care decisions for you when you can't make them.
-The kind of medical treatment you want or don't want.
-How comfortable you want to be.
-How you want people to treat you.

Five Wishes is available in 23 different languages and meets the legal requirements in 42 states and is useful in all 50. For a presentation on FIVE WISHES call 352-527-2020 or 866-642-0962

Here's what you can do with Five Wishes Online:

  • Create an account that gives you access to Five Wishes Online for 30 day.

  • Follow the prompts: check the appropriate boxes, select an option, or write a few sentences - and create a customized PDF that includes all the answers you provided. Or, print a customized Five Wishes (PDF) that includes your name, then fill it out by hand with a pen. Then all you have to do is sign the document and have it properly witnessed by two people, following the instructions that apply to your state.

  • Print the Five Wishes PDF in any of 26 languages.

  • Play video clips that describe each section
